Did you know that qualifying residents of Last Frontier Healthcare District who have paid their special property tax assessment are eligible to redeem a $150 voucher toward medical services at MMC?

To determine voucher eligibility, please follow these steps:
1. Obtain your parcel number from the Modoc County Assessor’s Office, (530) 233-6218.
2. Present your parcel number to MMC Patient Financial Services by phone at (530) 708-8819 to see if you qualify!

PLEASE NOTE: Vouchers are valid for two years, with an expiration date printed on the physical voucher. If you don’t plan to utilize your voucher, it may be applied to the medical expenses of someone in your household. Vouchers may not be sold or otherwise distributed to those outside of the unique property owner/dependent. The property owner/dependent will not be reimbursed for unused or expired vouchers. Lost vouchers CANNOT be replaced.
